Dental Insurance is different from Medical Insurance. Please review the insurance benefit booklet you received from your provider to better understand the benefits available as part of your insurance coverage. The patient payment portion covered for procedures varies depending on the coverage provided by your policy.

The patient is responsible for any deductibles and copays as instructed by the insurance carrier we participate with, at the time of service. If you have a dental insurance plan not listed above, we will be happy to process any and all of your paperwork necessary to file a dental claim. The patient is responsible for any fees at the time of service. If there is no insurance coverage, the patient is responsible for any fees at the time of service. Please let us know if you have questions about our financial policies or financing options prior to your treatment.


Payment Options


For your convenience, we accept payments made with cash, personal check, Visa, MasterCard, Discover, and American Express.


Cancellations


Kindly provide at least 48 hours’ notice for any appointment that needs to be rescheduled. Appointments canceled in less than 24 hours may be subject to the following cancellation charge:

  • $100/hour – Doctor cancellation
  • $50 – Hygiene cancellation

When booking a ZOOM video appointment, we require a $100 deposit. This deposit will be applied towards the total cost of your procedure, but is non-refundable for patient no-shows.
